ONLINE STUDENT EXIT FORM FOR B.TECH 2017 BATCH
All the outgoing B.Tech students of Batch 2017 have to complete the Online Student Exit forms. On the successful submission of the form, by each student, the Character certificate will be processed. The documents to be uploaded are as follows:
i) Program Exit Survey form (PES)
ii) Alumni Association Registration form (AA)
iii) Alumni Exit Survey form (AES)
iv) Self-Declaration form (SD)
v) Passport size Photograph with name and enrollment number (PH)

Instructions for filling the forms:
Students are advised to rename the file of each form, to be uploaded, as follows:
For example for a student with Enroll No: 2017BITE450, the file names of the documents to be uploaded are as follows:
i) For Program Exit Survey (PES) form, the name of the file for the above Enroll No will be
PES-2017BITE450
ii) For Alumni Association (AA) Registration form, the name of the file for the above Enroll No will be
AA-2017BITE450
iii) For Alumni Exit Survey (AES) form, the name of the file for the above Enroll No will be
AES-2017BITE450
iv) For Self-Declaration (SD) form, the name of the file for the above Enroll No will be
SD-2017BITE450
v) For Passport size Photograph with name and enrollment number, the name of the file for the above Enroll No will be PH-2017BITE450
For Passport size photograph the name and enrollment number should be put at the bottom of the photograph as shown in the sample.

Students are advised to first download these forms, fill them up and keep all these documents ready before filling the same details online and finally upload all the filled documents. Forms which are incorrectly filled and/or incomplete in any respect will not be processed.
